The Care Manager provides direct assistance to residents with daily living activities such as bathing, dressing, toileting, and feeding according to their care plans. They are also responsible for documenting services provided, monitoring resident safety, and reporting any changes in condition to the supervisor. Candidates must hold a Certified Nursing Assistant (CNA) or DHS certification and possess a High School Diploma. Experience working with the elderly population, particularly those with dementia, is preferred.
