Matrix Medical Network is seeking a dedicated Nurse Practitioner or Physician Assistant to join our team on a PRN (as-needed) basis, focusing on in-home health assessments in Emporia, Virginia. This role provides an exciting opportunity to contribute to patient care through various services, including health education and physician referrals.
Responsibilities
- Conduct in-home health assessments for patients.
- Provide health education and resources to enhance patient well-being.
- Facilitate physician referrals and communication for necessary treatments.
- Engage in case management to coordinate patient care.
- Deliver services in patients’ homes or in Skilled Nursing Facilities as required.
Qualifications
- Current certification as a Nurse Practitioner or Physician Assistant.
- Experience in conducting health assessments and case management is preferred.
- Strong communication skills to effectively educate patients and collaborate with healthcare teams.
- Ability to work autonomously and manage a flexible schedule.
