Job Summary
The ACT Community Support Specialist plays a pivotal role in providing comprehensive support to individuals within the community, fostering engagement and facilitating access to essential services. This full-time position is dedicated to empowering clients by aiding their recovery and improving their quality of life through tailored interventions.
Responsibilities
- Engage with clients to understand their unique needs and develop customized support strategies.
- Assist clients with daily living activities and help them navigate available resources.
- Collaborate with multidisciplinary teams to ensure holistic service delivery.
- Conduct regular assessments to monitor client progress and adjust care plans as needed.
- Provide crisis intervention and support during emergencies.
- Document client interactions and maintain accurate records in line with confidentiality standards.
Qualifications
- Degree in Social Work, Psychology, or a related field is preferred.
- Experience in community support or mental health services is highly beneficial.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team-oriented environment.
- Familiarity with relevant community resources and support systems.
- Valid driver’s license and reliable transportation may be required.
