Job description
The Care Extender will collaborate with community partners and stakeholders to develop support strategies and address systemic challenges. In addition, this role will offer direct assistance to care management teams by linking foster care members and their families to vital community resources. Eligible candidates should hold a high school diploma or equivalent and have a minimum of 2 years of relevant experience. It is essential for applicants to live in Henderson County, North Carolina, to meet the hybrid work criteria.
