**Case Manager**
The Case Manager is responsible for organizing and overseeing interdisciplinary care plans, handling discharge planning, and ensuring effective communication among patients, caregivers, and the healthcare team. This role includes monitoring treatment objectives, recording care activities in accordance with CMS guidelines, and recognizing obstacles that may impede effective care transitions. Applicants are required to possess a professional license as a Registered Nurse, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist. A minimum of 2 years of experience in social work or case management in an inpatient environment is highly preferred.
