**Hospice RN Case Manager**
The Hospice RN Case Manager is responsible for orchestrating and overseeing end-of-life care by conducting thorough patient evaluations and crafting personalized care strategies. This role involves working closely with a diverse healthcare team to deliver treatments, alleviate symptoms, and offer emotional assistance to both patients and their families. Applicants must possess a valid RN license from Washington State and certification in BLS. A minimum of 2-3 years of experience in healthcare is essential, with a strong preference for candidates who have at least 1 year of experience in hospice or home health settings.
