Job Summary
The position entails performing clinical evaluations and organizing care for individuals experiencing mental health and social difficulties. The successful candidate will work alongside multidisciplinary teams to provide thorough support while ensuring precise documentation of clinical information.
Responsibilities
- Conduct clinical assessments of clients to identify mental health and social challenges.
- Coordinate care plans in collaboration with various healthcare professionals.
- Maintain accurate and detailed clinical records for all client interactions.
- Work effectively within multidisciplinary teams to deliver comprehensive support to clients.
Qualifications
- Master’s degree in Social Work is required.
- Possession of a valid LCSW license in the state of Oregon.
- Proven experience in conducting clinical assessments.
- Strong communication skills to facilitate collaboration with team members and clients.
