Job description
The Licensed Clinical Social Worker will evaluate, provide counseling, and guide patients with complex medical needs through the creation of tailored treatment plans. Additionally, the role involves connecting patients to resources and promoting behavioral changes within their households and community environments. Applicants must have a Master’s degree in Social Work and be licensed as an LCSW in their respective state. Strong skills in planning, organization, written communication, and interpersonal communication are essential for success in this position.
