Develop personalized treatment plans for medically complex patients by providing assessments, counseling, and guidance. Support patients in adapting to disability, connect them with resources, and apply behavior modification techniques. A Master’s degree in Social Work and an active LCSW license in the state are required. Applicants should demonstrate strong skills in planning, organization, writing, and communication.
