**Job Title: Nurse Practitioner**
The Nurse Practitioner will deliver thorough follow-up care for patients after they have been discharged from the hospital via telephone and video consultations. Key responsibilities encompass medication reconciliation, evaluating symptoms, and facilitating coordination of care across different healthcare environments to enhance patient outcomes. Applicants must hold certification as a Nurse Practitioner in Adult, Family, or Geriatric specialties and possess a valid New Jersey license. A minimum of 5 years of clinical experience post-graduation in Primary Care or Geriatrics is mandatory.
