Job description
**Clinical Team Manager**
The Clinical Team Manager is responsible for supervising patient care operations, liaising with healthcare professionals, and guaranteeing the creation and execution of tailored patient care plans. This role also involves managing direct care personnel, performing OASIS assessments, and promoting effective communication among the care team, patients, and physicians. Applicants must possess an associate degree in a nursing-related discipline along with a current, unrestricted RN license. A minimum of two years’ experience in home health care with expertise in OASIS and a valid driver’s license are necessary.
