**Job Title: Hospice RN Case Manager**
The Hospice RN Case Manager is responsible for performing thorough patient evaluations and creating tailored care plans to guarantee high-quality support during the end-of-life phase. This role includes offering clinical guidance to nursing personnel and fostering effective communication among physicians, patients, and their families. Applicants must possess a valid RN license that is in good standing with the state nursing board. Prior experience in hospice care or a similar healthcare environment is advantageous, along with excellent clinical assessment abilities and communication skills.
