Job Summary
This position seeks a highly skilled and compassionate Hospice RN Case Manager to provide essential care and support to patients and their families. The ideal candidate will play a crucial role in managing patient care, ensuring compliance with treatment plans, and enhancing the quality of life for those in hospice care.
Responsibilities
- Conduct comprehensive patient assessments to develop personalized care plans.
- Coordinate with healthcare professionals to deliver high-quality hospice services.
- Provide education and support to patients and families regarding care options and end-of-life issues.
- Monitor patient progress and make adjustments to care plans as necessary.
- Maintain accurate documentation and records of patient care and communications.
- Serve as an advocate for patients’ rights and wishes throughout the care process.
Qualifications
- Registered Nurse (RN) with a valid nursing license.
- Experience in hospice, palliative care, or similar clinical settings is preferred.
- Strong communication and interpersonal skills.
- Ability to work collaboratively in a team-oriented environment.
- Compassionate approach to patient care and end-of-life issues.
