The role involves delivering thorough care coordination for children and young adults experiencing significant mental health challenges through High-Fidelity Wraparound methodologies. Responsibilities include serving as a link to community resources, ensuring precise clinical documentation, and assisting in the development of service plans. Candidates should possess a high school diploma coupled with over 3 years of relevant experience, an associate or bachelor’s degree with a minimum of 2 years of experience, or a master’s degree accompanied by at least 1 year of experience in complex case management. It is essential that applicants demonstrate expertise in behavioral health and exhibit the ability to effectively navigate local systems that support children and families.
