Job Summary
The position involves acting as a liaison for patients, families, caregivers, physicians, and community stakeholders, facilitating appointments, access to services, and comprehensive care throughout the healthcare spectrum. The role emphasizes collaboration with various multidisciplinary teams to enhance patient education, ensure safe care transitions, and address community health needs while monitoring outcomes for quality improvement.
Responsibilities
- Serve as the primary contact for patients and their families, ensuring effective communication and coordination.
- Assist in scheduling appointments and arranging access to healthcare services.
- Work closely with multidisciplinary teams to provide patient education and support.
- Facilitate safe transitions of care for patients.
- Identify patients at risk and assess community healthcare needs.
- Monitor and analyze healthcare outcomes to drive improvements in care delivery.
Qualifications
- Current and valid Ohio Licensed Practical Nurse or Registered Nurse license.
- Fluency in Pennsylvania Dutch.
- Demonstrated cultural competence and a strong commitment to serving the community.
- Current BLS certification is mandatory; ACLS certification is also required or must be pursued.
- Strong organizational skills and adaptability in a fast-paced environment.
- Excellent communication abilities and capacity to prioritize tasks effectively.
- Willingness to travel as required for the role.
