The Hospice RN acts as the central liaison for patients and their families, offering expert nursing assistance and managing symptoms based on personalized care plans. This role involves collaboration with a multidisciplinary team to deliver comprehensive emotional, physical, and spiritual support, while ensuring precise clinical documentation is maintained.
Responsibilities
- Provide skilled nursing care and symptom management to patients in hospice settings.
- Develop and implement individualized care plans in collaboration with the care team.
- Support patients and families emotionally and spiritually throughout the hospice journey.
- Maintain precise and accurate clinical documentation of patient care delivered.
Qualifications
- Must be a Registered Nurse with a minimum of one year of experience in acute care, oncology, long-term care, hospice, or home health.
- Possess a current and valid nursing license.
- Hold a valid CPR certification.
- Have a valid driver’s license and reliable transportation.
