Job Summary
The Registered Nurse Care Manager specializes in delivering comprehensive behavioral and physical health services according to the Assertive Community Treatment model. This role is critical in supporting adults facing serious mental health challenges through various healthcare interventions.
Responsibilities
- Oversee medication management for clients to ensure effective treatment adherence.
- Provide crisis intervention to individuals in need, enhancing their overall wellbeing.
- Conduct case management duties, coordinating care and resources for patients.
- Engage in outreach services to connect with and support adults with serious mental illnesses.
Qualifications
- An Associate’s degree in Nursing is mandatory.
- A current Registered Nurse license issued by the State of Michigan is required.
- At least one year of experience working with clients who have mental illness.
- A valid driver’s license is essential to perform job duties effectively.
