Job Summary
The Home Care Case Manager plays a crucial role in organizing and delivering home care services prescribed by medical professionals. Responsibilities
- Coordinate and deliver home care services as per physician orders.

Qualifications
- Active Ohio Registered Nurse license is mandatory.
- A minimum of 1-3 years of clinical experience is preferred, ideally in an acute care environment.
- Demonstrated proficiency in IV skills is required.
- Ability to perform physical tasks, including lifting weights of up to 40 pounds.
