Provide skilled nursing and rehabilitative care to adult and geriatric clients in their homes, including assessment, triage, medication reconciliation, patient education, and wound care. Document care, monitor patient safety and condition, and independently manage urgent clinical situations while coordinating with the multidisciplinary team. Applicants must hold a current nursing license in good standing and graduate from an accredited nursing program. The role requires OASIS documentation experience and at least one year of recent hands-on medical experience with adult or geriatric patients in an eligible care setting within the past two years.
