Job Summary
The Manager will be responsible for overseeing home services and coordinating care efforts. This role includes supervising staff, facilitating onboarding processes, and developing tailored service plans for residents. The Manager will also regularly engage with residents to offer health education, advocate on their behalf, and monitor the implementation of care plans.
Responsibilities
- Supervise home services and ensure effective care coordination.
- Manage staff onboarding and training procedures.
- Develop individualized service plans to meet resident needs.
- Establish ongoing communication with residents for health education and support.
- Advocate for residents and ensure they receive appropriate care.
- Monitor and evaluate the implementation of care plans.
Qualifications
- Degree in allied health or nursing is required.
- At least one year of experience in community-based services.
- A minimum of two years of supervisory or management experience in care settings.
