Job description
**Job Title: Geriatric Care Provider**
Deliver preventive primary health services and manage chronic health conditions for adults aged 65 and older, adhering to a value-based care framework. Work in conjunction with a multidisciplinary team that includes nurse practitioners, physician assistants, medical assistants, and social workers to ensure comprehensive coordination of patient care. Applicants should possess Board Certification or be Board Eligible in Internal Medicine or Family Medicine. While prior experience in geriatrics, value-based care, or HEDIS is advantageous, it is not a mandatory requirement.
