The Registered Nurse will oversee patient care from the point of admission to discharge, which includes conducting OASIS assessments and crafting personalized care plans. This role involves delivering skilled nursing services and working with an interdisciplinary team to achieve optimal patient outcomes. Applicants must have an active RN license in Ohio and a valid driver’s license, along with dependable transportation. While experience in home health and familiarity with Medicare regulations is preferred, it is not mandatory.
