The Case Manager is tasked with delivering immediate crisis intervention, support, and educational resources to adults with Serious Mental Illness (SMI) and their families. This role involves conducting needs assessments, managing referrals, and facilitating seamless care across various community services. Applicants are required to fulfill behavioral health technician qualifications, having either a high school diploma accompanied by four years of experience or a relevant degree with equivalent experience. Furthermore, possession of a valid driver’s license, dependable transportation, and a fingerprint clearance card is essential.
