Job Summary
The Case Manager for Integrated Community Mental Health Services (ICMS) plays a vital role in supporting individuals living with mental health challenges. This position involves assessing client needs and coordinating appropriate services, ensuring they receive comprehensive care tailored to their personal circumstances.
Responsibilities
- Conduct thorough assessments to determine the needs of clients.
- Develop personalized service plans in collaboration with clients and their support networks.
- Coordinate with various providers and community resources to facilitate access to comprehensive services.
- Monitor and evaluate client progress and adjust care plans as necessary.
- Provide advocacy and support to clients navigating community systems.
- Document all interactions and maintain accurate client records.

Qualifications
- Bachelor’s degree in social work, psychology, or a related field.
- Experience in case management or related services is preferred.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
