Job Summary
The Hospice RN functions as the main liaison for patients and their families, offering expert nursing services and managing symptoms in either home or facility environments. They engage with a multidisciplinary team to create tailored care plans and extend emotional and spiritual assistance throughout the hospice experience.
Responsibilities
- Provide skilled nursing care and manage patient symptoms.
- Collaborate with an interdisciplinary team to formulate individualized care plans.
- Offer emotional and spiritual support to patients and families during the hospice process.
Qualifications
- Current Registered Nurse license.
- Minimum of one year of experience in fields such as acute care, oncology, long-term care, hospice, or home health.
- Valid CPR certification.
- Possess a valid driver’s license and have reliable transportation.
