Responsibilities
- Deliver healthcare services including treatment and health education.
- Provide referrals to physicians as needed.
- Engage in case management to oversee patient care.
- Work with patients either in their home environments or in Skilled Nursing Facilities.
Qualifications
- Must be a licensed Nurse Practitioner or Physician Assistant.
- Experience in case management and delivering healthcare in various settings is preferred.
- Ability to work both independently and as part of a multidisciplinary team.
- Strong communication and patient management skills.
