Job Summary
The role involves providing essential care to residents in assisted living or memory care settings. The candidate will administer medications, perform assessments, observe any changes in residents’ conditions, and meticulously document all care provided. The position emphasizes maintaining personalized care plans and engaging in interdisciplinary team meetings.
Responsibilities
- Deliver direct care to residents within assisted living or memory care environments.
- Administer prescribed medications and conduct thorough assessments of residents.
- Monitor any shifts in residents’ health status and document care accurately.
- Administer and uphold individualized care plans for each resident.
- Take part in interdisciplinary meetings to discuss resident care strategies.
- Measure and record vital signs and other health indicators.
- Support and train direct care staff, focusing on best practices and fall prevention measures.
Qualifications
- A high school diploma or GED is required.
- An active Licensed Practical Nurse (LPN) license in the relevant state.
- Current CPR certification is mandatory.
- A minimum of two years of experience in a related field is necessary.
- Familiarity with medical terminology, medical records management, and clinical procedures is essential.
- Ability to fulfill the physical demands of the role.
