Job Summary
The position of Nurse Practitioner or Physician Assistant (PRN) involves conducting in-home health assessments to support the healthcare needs of patients in New Rochelle, New York. This role is critical for delivering effective treatment, health education, and managing referrals for physician and case management services.
Responsibilities
- Conduct comprehensive health assessments for patients in their residences or at Skilled Nursing Facilities.
- Provide health education to patients and their families to enhance understanding of treatment plans.
- Facilitate referrals to physicians and case management services as needed.
- Document patient evaluations and progress in accordance with organizational policies.
- Collaborate with a healthcare team to ensure continuity of care for patients.
Qualifications
- Must hold a valid Nurse Practitioner or Physician Assistant license.
