Responsibilities
- Conduct in-home health assessments for patients, ensuring thorough evaluations.
- Provide treatment and health education tailored to individual patient needs.
- Facilitate physician referrals and coordinate case management for patients.
- Engage with patients in various settings, including residential homes and Skilled Nursing Facilities.
Qualifications
- Must hold a valid Nurse Practitioner or Physician Assistant license.
