Job description
The Social Worker Case Manager is responsible for evaluating the biopsychosocial needs of patients, while also collaborating with the interdisciplinary team to develop and implement care plans. Additionally, this role involves managing discharge planning and offering guidance on the emotional, social, and financial consequences of health conditions. Applicants are required to have a Master’s degree in Social Work along with a minimum of one year of experience in clinical care or case management. A Basic Life Support (BLS) certification from the American Heart Association must be obtained before commencing the position.
