Job Summary
The Clinical Care Coordinator acts as the main patient guide and clinical organizer for intricate subspecialty programs. This position involves streamlining care among diverse teams and assisting healthcare providers in managing comprehensive care as well as promoting the growth of programs.
Responsibilities
- Coordinate patient navigation through complex subspecialty care.
- Work collaboratively with multidisciplinary teams to ensure seamless care delivery.
- Support providers in executing comprehensive care management.
- Contribute to the expansion and development of clinical programs.
Qualifications
- Completion of an accredited medical assistant program or equivalent apprenticeship.
- Possession of a National Provider BLS certification.
- Minimum of 3 years of experience in healthcare and office settings.
- Proficient in EMR systems and Microsoft Office applications.
